The fee is what is listed above plus a $12 processing fee.
A customized requisition form will be mailed to you upon receipt of your order.
You take the form to the selected patient service center (Alliance Lab in Sedona, or your choice) where the laboratory technician will draw a blood sample for your test ordered.
The results of your test will be mailed or faxed to you within 10 business days, or you may view your results online.
Remember to eat or drink nothing but water (but plenty of it) the 12 hours before the blood is drawn.
